Book a Telephone Case Conference

GPs can book a telephone case conference with Dr Tashi Zhaxiduojie (FRACP, PhD) to discuss patient management. Simply fill in the details below and select an available time slot.

How It Works

Step 1: GP Requests Conference — The GP submits a telephone case conference request via the online form.

Step 2: HeartWise Confirms Date — HeartWise Cardiology reviews and confirms the scheduled date and time.

Step 3: Telephone Case Conference — Dr Tashi conducts the telephone case conference with the referring GP.

Please note

This telephone case conference requires three participants: Dr Tashi Zhaxiduojie (cardiologist), a GP from the referring practice, and an allied health professional or practice nurse. Please ensure all three participants are available at the scheduled time.

It is the referring doctor's responsibility to ensure all Medicare billing requirements for the case conference are met.
